On behalf of our physicians, nurse practitioner and staff, welcome to Northwest Women's Consultants. From its early beginnings, our practice has strived to provide high quality care while maintaining compassionate, caring relationships with our patients. Mostly through word-of-mouth from our patients, our practice has grown and grown---from two physicians to seven physicians, a nurse practictioner and extensive ancillary staff. We enjoy taking care of women at all stages in their lives--we often take care of an entire family--daughters, mothers, grandmothers; this is one of the best parts of our job! Whether you need a pap smear or birth control pills, help with your menstrual cycles or menopause, advice on getting pregnant or what to do when you become pregnant, or surgery---we are here to help educate you about your health care and any treatments you might need. Our doctors, nurses and staff will work as a team to guide you through that process with the courtesy and understanding you deserve.

- Please call 847-394-3553 to schedule an appointment.
- Please check the list of insurance plans accepted by our practice
- Please fill out print out the health history form and patient information form and fill
fill out prior to your visit. Bring the forms you have filled out to your visit with you, or
feel free to fax them to our office (847-394-3574).
- If you are transferring your obstetrics care or are seeing us for a problem previously
diagnosed by another physician or have had gynecologic surgery in the past, please
have a copy of your medical records with you at your appointment or have this sent
to us prior to your visit. It may take your previous doctor's office a few weeks to
compile and send your records, so please call them well in advance.

- A copy of your insurance card is REQUIRED at each visit. If you do not have your
insurance card with you at your visit, we require payment at the time of service.
We will then be happy to provide you a receipt to submit to your insurance
company.
- Yearly, we will ask that you update your health history form and patient
information form. You may print them out from this website and either bring with
you to your visit or fax to our office.

- All co-pays are due at the time of your visit.
- If you are unable to make your scheduled appointment, please call us in advance. There is a fee assessed for patients who do not show up for an appointment without calling.
